Local Health Departments Annual Program Plan

The Program Plan serves as the data collection tool for West Virginia's local health departments.  Information collected from the Program Plan includes: projected budget documents, personnel, web-based and printed profiles, Local Health Department contact book information, emergency contact information, environmental health inventory, community health needs assessment, threat preparedness planning contacts, and work plans related to performance-based public health standards.
